In-Home Primary Care

For patients who don’t have a doctor, we can bring compassionate medical professionals into their home. This includes check-ups wellness visits, telehealth and other routine services designed to improve the health of older adults.

Care Coordination

Our health coaches and social workers work together to ensure your social and medical needs are not just met, but are well-coordinated. We ensure your entire care team stays connected to make sure there are no gaps in your health plan.

Clinical Health Coach

Our health coaches work with our nurse practitioner to create a personal healthcare plan-of-action that aims to keep you feeling good and independent at home. Throughout your relationship with GrayHawk, your health coach will be there to provide education, support and encouragement. Our health coaches offer continuous guidance through in-person and online check-ins to make sure you are staying on the right track to better health.

Post-Hospital Care

When you come home after being in a hospital, skilled nursing facility, or an acute rehab, our team will be there to provide the extra care that you need. Our nurse practitioners and health coaches will visit you soon after you return home to make sure you have the proper medication, your follow-up care is scheduled and all of your healthcare needs are met.

Social Services

We provide more than medical care. Though our community partnerships, we will connect you to important support services such as home-delivered meals, transportation, community support resources and home modifications to assist with mobility.

Betty’s story

I’m currently working with a patient who needs a knee replacement but can’t have the procedure until her diabetes is under control.

We identified the key barriers to better diabetes management and are starting with small, simple changes. First with a more balanced meal plan and then remote blood sugar monitoring. When I see that her readings are high, I can reach out and coach her. We speak often and I try to motivate her with positive support and encouragement. She is on her way to achieving her goal of a new knee and a happier, less stressful life.
