We Surround Members with a Whole-Person Approach to Health and Wellness
In-Home Primary Care
For those who don’t have a primary care doctor, we can bring compassionate medical professional services into their home. Our in-home services include check-ups wellness visits, tele-health and other routine services designed to achieve and maintain members’ optimal levels of health.
Care Coordination
GrayHawk health coaches and social workers work together to ensure members’ social and medical needs are not just met, but are well-coordinated. The entire care team stays connected and works to ensure there are are no gaps in care plans and needed treatment.
Wellness Coaches
Our health coaches work with our nurse practitioners to create individual member healthcare plans that aim to keep members well and living independently at home. As an ongoing part of our member care services, GrayHawk health coaches provide education, support and encouragement to keep members engaged and active in achieving their health goals. GrayHawk coaches offer continuous guidance through in-person and online check-ins to make sure members are staying on the right track to better health.
Post-Hospital Care
Following any care members receive in a hospital, skilled nursing facility, or an acute rehab facility, our nurse practitioners and health coaches visit members soon after their return home to make sure they have the proper medication, that follow-up care is scheduled and all of their healthcare needs are met.
Social Services
We provide more than medical care. Should it be needed, though our community partnerships we connect members to important support services such as home-delivered meals, transportation, community support resources and home modifications to assist with mobility.
Closing the Gaps
GrayHawk’s goal is to keep patients healthy and reduce avoidable ER and hospital visits. Our wrap-around care model enables us to proactively meet patient needs in their homes through in-person visits or tele-health sessions. In addition, our steady care and support boosts member confidence over time, so they take more control of their health and lives.
